What is your admission process like?
The admission process is fairly simple and only takes about 20 minutes to complete.

What is the difference between Personal Care facilities, Assisted living facilities or Intermediate Care and Skilled Nursing facilities?
Assisted Living Residences offer changing services so that a resident may advance to a nursing home level of care when necessary. When living in a Personal Care Home, a person whose health needs become too great will be transferred to a skilled nursing facility. A skilled nursing facility is usually temporary in nature and is focused on skilled 24 hour medical rehabilitation that is intended to prepare the resident to return to more stable independent care. Assisted living or Intermediate care services typically provide assistance with bathing, dressing, grooming, medications, and meal preparation.

What medical paperwork will I need?
Before admission, the Eastern Star Home must receive a History & Physical (H&P) either from the primary care provider of the resident or from the current facility in which they may live.

What other paperwork will I need?
A copy of the Power of Attorney, Living Will and Insurance Cards

What happens when the patient’s funds run out?
If it appears that funds are running out for the resident, it is important to communicate with us so that we may assist in other arrangements while the POA starts the process of applying for Medicaid.

What is a private pay facility?
A private pay facility refers only to fees associated with room and board. Depending on the type of care and the individual covered, you may be able to pay for skilled nursing with long-term care insurance or personal funds.

Do you have a waiting list?
Eastern Star is always accepting resident applications with over 40 beds including accommodations licensed by the Commonwealth of Kentucky as an 18 bed Intermediate Care Facility and a 24 bed Personal Care Home.

How do I transition from the hospital to your care facility?
It’s easy. Just call us or apply online and we can start the process! You can also ask your discharge planner or case manager at the hospital to contact us directly on your behalf to ensure a smooth transition to our center.

What to do when you are not covered by Medicaid/Medicare but you need an elder care facility?
Eastern Star Home is a non-profit care facility that strives to offer high quality affordable private pay care for individuals who do not qualify for medicaid or medicare facility services.
